What insurance company provides the coverage for the Student Health Insurance Policy?

Does the Student Health Insurance Policy meet the Health Care Reform/Affordable Care Act Requirements?

YES! The policy meets all Healthcare Reform/Affordable Care Act (ACA) and State of Ohio required mandates, and offers coverage specifically tailored to 51社区 student needs. The policy offers students and their dependents a comprehensive commercial health insurance policy, with the convenience of having the premium(s) billed to their student account. 

Where can I locate information regarding the Student Health Insurance Policy?

Is the Student Health Insurance Policy only for treatment received while on Campus?

No. The Student Health Insurance Policy is a commercial policy provided by United Healthcare Student Resources, and can be used nationwide. The policy offers an extensive network of Preferred Providers through the UHC Choice Plus provider network. To locate providers, visit our  with the insurance company and select "Find Providers" under the image of the Athens campus Convocation Center. You can then choose to search for a medical provider or facility, or a behavioral health provider. Once selected, you can customize your search based on your location and desired provider preferences. 

Does the Student Health Insurance work only while classes are in session?

No. However, our predetermined coverage periods coincide closely with the dates of our semesters. The Student Health Insurance Policy is a 1-year commercial policy provided by United Healthcare Student Resources, and is re-negotiated each academic year.

The policy year runs August 16th through August 15th of the following year.

Can a student or dependent enroll in the Student Health Insurance Policy at any time?

Voluntary enrollment is available during the open enrollment periods at the beginning of each semester. Students may submit an enrollment form after registering for classes for the current coverage period.

Dependents are eligible to voluntarily enroll only for coverage periods in which the student is also enrolled in the Student Health Insurance Policy.

A Qualifying Life Event (QLE) allows a student to enroll in the Student Health Insurance (for which they meet the eligibility requirements) on a monthly/weekly prorated basis until the start of the next predetermined coverage period. If you have experienced a QLE, please reach out to us via email or phone at 740-593-1931 for a quote or to enroll. 

Refunds and reduced premium amounts cannot be provided for late enrollment or early termination.

Can I voluntarily purchase the Student Health Insurance online?

No. Student Health Insurance can only be purchased by submitting a completed enrollment form to the Student Health Insurance Office during Open Enrollment each semester. Qualifying Life Event enrollments are processed as needed.

Completed enrollment forms must be emailed.  

Can students purchase insurance for only a portion of a coverage period?

No. The Student Health Insurance can only be purchased for the predetermined coverage periods. There is no prorated premium amount for late enrollment or early cancellation. (Does not apply to QLE enrollments)

How can I obtain enrollment forms?

Enrollment forms may be found on this website or by emailing your request and PID number.

Where can I send my enrollment form?

Currently, enrollment forms are only accepted by email.

 

How do I enroll in the University Health Insurance plan if I have already submitted an online waiver?

Students can voluntarily enroll in the Student Health Insurance during open enrollment periods at the beginning of each semester by submitting a completed enrollment form prior to the posted deadline.

Why do dependent enrollment forms need to be submitted each semester?

Eligibility for the Student Health Insurance is determined each semester by student registration. The University system operates on an academic calendar by the semester. It is not possible to process enrollment forms for future semesters due to eligibility verification that must be performed.

How can I determine what physician鈥檚 and facilities are In-Network with the Student Health Insurance Policy?

There is a Provider Locator feature on the insurance website that can be used to locate In-Network providers. The network is United Healthcare Choice Plus, which provides a large selection of providers nationwide. Visit and select "Find Provider" on the link funder the picture of campus. United Healthcare Student Resources Customer Service can be reached at 1.800.767.0700 to assist you in determining if a provider is In-Network.

*Please Note: Students with the Student Health Insurance are required to seek treatment at Campus Care in Athens first if they are within 30 miles of the Athens campus and Campus Care is open.

Where can I obtain my insurance ID card?

United Healthcare Student Resources is a paperless company does not automatically mail ID cards to students. Students will be prompted to create an account online with to manage their benefits and obtain their ID card. The online account allows students to print a copy of their ID card, upload a digital copy to a smartphone or tablet, or request United Healthcare Student Resources to mail them a hard copy of the ID card.

 

Does the Student Health Insurance Policy include Dental Insurance?

No. Dental treatment is a policy exclusion and has no payable benefits for insureds age 18 and over.

Pediatric dental benefits are included in the Student Health Insurance Policy for insured persons under the age of 18. There are specific providers you must receive treatment from in order for the insurance to make a payment on the claim. Please see the Pediatric Dental provisions in the policy certificate.

*Injury to a natural sound tooth by trauma is considered a payable benefit with the Student Health Insurance policy for all insured persons. There is no preventative care included for insureds over the age of 18.

Can I Purchase Dental coverage?

United Healthcare Student Resources offers an optional dental policy for purchase at beginning of Fall semester. The policy can be purchased through the insured鈥檚 online account with United Healthcare Student Resources, on their  or by contacting them directly at 1.800.767.0700. Students may purchase the optional Dental coverage without purchasing the student health insurance.
