Vaccination Information
51社区 requires that all residential students disclose their current meningitis and hepatitis B vaccination status prior to residing on-campus.
Meningitis and Hepatitis B
The Ohio Revised Code (ORC) Section 1713.55 states that beginning with the academic year that commences after July 1, 2005, an institution of higher education shall not permit a student to reside in on-campus housing unless the student (or the student鈥檚 parent if the student is younger than 18 years of age) discloses whether the student has been vaccinated against meningococcal disease and hepatitis B by submitting a meningitis and hepatitis B vaccination status statement.
Please note that the state of Ohio law does not require vaccination of the student, nor does it require the institution to provide or pay for vaccines. The law requires only the disclosure of vaccination status of the student.

By completing the Vaccination Information Form, the student will disclose their vaccination status and acknowledge that they have read and understood the risks associated with meningitis and hepatitis B, and the information listed on the as required by ORC Section 3701.133.

The form should be completed by July 1, but no later than prior to arriving to campus for check in. Students will not be permitted to check in until the form is complete. Students can revisit this contract at any time on your page to update their vaccination information for the upcoming academic year.
